**Growing Up with NF: Pediatric-to-Adult Transition**
NF and schwannomatosis require lifelong monitoring, and the shift from pediatric to adult care is one of the most common places that continuity breaks down. The UCSF NF Center is built to prevent that gap: our pediatric and adult programs are co-directed as a single team, so patients move from one to the other without losing their care history or support network.
**Who this is for**
- Adolescent and young adult (AYA) patients with NF1, NF2-related SWN, or non-NF2 SWN moving from pediatric to adult care
**What transition support includes**
- A structured hand-off between pediatric and adult NF teams, coordinated by our Co-Directors
- Continued access to the Sheri Sobrato Survivorship Program (with the Anna Kathrene Smith Fund)
- Gordon Murray Caregiver Program support for families navigating the shift
- Neuro-Cognitive Service — joint clinic support for patients with cognitive or learning effects of NF-1
- YABS (Young Adult Brain Tumor Support) group community for NF patients with brain tumors
- Dedicated social work support through coordination between our pediatric and adult programs
